New Patient Evaluation in Southlake, TX
Your first appointment at MindMED Behavioral Health is a comprehensive psychiatric evaluation lasting 60–90 minutes. Dr. Fredes will take time to understand your complete history, current concerns, and treatment goals.
Before your visit: You’ll receive intake forms via email to complete at your convenience. This helps us make the most of our time together.
Insurance patients: We are in-network with Aetna, Blue Cross Blue Shield of Texas, Cigna/Evernorth, Oscar Health, and United Healthcare/Optum. Your cost will depend on your plan’s copay, coinsurance, and deductible. We recommend calling your insurer to verify your benefits before your visit.
Self-pay patients: $400 for initial evaluations. We provide superbills for out-of-network insurance reimbursement.
Location: Our office is at 536 Silicon Drive, Suite 102, Southlake, TX 76092. We also offer telehealth appointments for patients in Texas, New York, and Virginia.
